This Telehealth Consent (“Consent”) applies to telehealth services provided by licensed medical professionals affiliated with Neuday LLC through its secure, HIPAA-compliant digital platform. '

Overview of Telehealth
Telehealth allows Neuday’s licensed providers to evaluate and treat patients remotely using secure technologies, including dynamic intake forms, live video consultations, and electronic messaging. These technologies are protected by industry-standard security protocols to safeguard your privacy and personal health information.
Benefits of Telehealth

Risks and Limitations of Telehealth

Telehealth is not a substitute for in-person medical evaluation or emergency care

Your provider may not have access to complete medical records or be able to conduct a physical exam

Some medical conditions may require in-person care or diagnostic testing

Technical issues (e.g., poor internet connectivity) may affect the quality of your consultation

Unauthorized access to your data, while unlikely, is a risk inherent in all digital systems

State-specific laws may limit the scope of telehealth services, including prescribing
Medical Disclosure Requirements
To ensure accurate diagnosis and treatment, you agree to:

Disclose your complete medical history, diagnoses, medications, and supplements

Provide accurate, up-to-date information during your consultation

Inform your provider of any changes in health status, adverse reactions, or new medications
Failure to provide full and accurate information may result in inappropriate treatment or medical complications.
Provider Scope and Consent to Treatment

You may be treated by a licensed medical doctor, nurse practitioner, or physician assistant

You consent to receiving treatment from these professionals via telehealth

Telehealth consultations will not be recorded

No treatment or medication is guaranteed; your provider will determine appropriateness based on clinical judgment

You may be referred to an in-person provider if telehealth is not appropriate
Use of Compounded Medications
If medically appropriate and permitted under Texas law, your provider may prescribe compounded GLP-1 medications (e.g., semaglutide or tirzepatide) sourced from licensed Texas-based pharmacies. These medications:

Are regulated by the Texas State Board of Pharmacy

Are not FDA-approved, but are subject to strict compounding standards

Undergo third-party quality testing for safety and purity

Note:

Ozempic®, Wegovy®, Zepbound®, and Mounjaro® are FDA-approved, non-compounded medications and registered trademarks of their respective companies. Neuday does not dispense these branded medications.
Emergency Situations
Telehealth is not appropriate for medical emergencies. If you experience an emergency or an allergic reaction:

Visit your nearest emergency department
Withdrawal of Consent

You may withdraw your telehealth consent at any time by contacting us at info@neuday.com

Withdrawal will not affect care provided prior to notice

Continuing to use Neuday’s services constitutes ongoing agreement with this Consent
Privacy and Communication

Your personal and health information will be handled in accordance with Neuday’s Privacy Policy

All data is transmitted using secure, encrypted technology

You acknowledge that electronic communication may still carry minimal risks
Financial Responsibility

You are responsible for all costs related to your treatment and medications

Neuday does not accept insurance or Medicare reimbursement

Prescriptions are fulfilled through partner compounding pharmacies and shipped directly to you
